We know that a cancer diagnosis can affect many areas of life, for both you and your loved ones. Survivorship care is an important part of your cancer journey and provides support for the physical, emotional and practical needs of people living with cancer.

The clinic is made up of a team of experts who’ll provide you with information and resources to meet your unique needs as a cancer survivor. We work to improve your quality of life, longevity and general well-being and help you manage issues you may face during or after treatment. Services are available at any point in your cancer journey, and visits are available in-person or telehealth (video).

What to Expect

A visit with survivorship experts includes:

  • A personalized discussion of your physical, social, emotional, practical and spiritual concerns.
  • Information and tips for healthy living during and after cancer treatment, including nutrition, exercise, tobacco cessation and emotional well-being.
  • Resources and connection to other health care services, such as counseling, oncology rehabilitation and symptom management.

After your cancer treatment ends, we’ll give you a document called a Treatment Summary and Survivorship Care Plan that includes information about:

  • Your cancer treatment history
  • Potential side effects related to your cancer treatment and how to manage these symptoms
  • How often you’ll have visits with your cancer team for cancer screening
  • Recommendations for additional health and cancer screenings

This information will be shared with your other health care providers so they have a record of your cancer history.
